Our service delivery model within MSH at Home is based around the Buurtzorg Model from the Netherlands. This model places the client at the heart of service delivery, with each client having a 'Named Nurse' who will plan the care activities for that client, in partnership with the client and/or their family. The Named Nurse then supports and supervises a team of Health Support Personal Assistants (HSPAs) who deliver these activities, to encourage and maintain the client living independently, but safely, within their own home.
To ensure consistency, each Named Nurse will build a team of HSPAs within a geographical setting, to support the clients in that area. Currently we support a number of Islington residents living in the Angel area, and the team above assist those clients to enjoy the many social and recreational facilities that Angel has to offer.
